Indicate any further information you have that may help us locate the incident in our records.
Fill out the entire form; incomplete forms will not be processed. All Fire Incident Reports are available for release after they have been reviewed and approved by the Fire Chief. Please allow 7-10 business days for your request to be processed. If you chose to pick up the report, we will notify you when it is ready.
